We are proud to offer a CARF-accredited Patient Rehabilitation Program (PRP) designed to support individuals facing mental health challenges, life-altering circumstances, and the difficult transition of reentering society. Our mission is to provide compassionate, community-focused care that promotes healing, personal growth, and independent living.

What is PRP?

Our PRP is a structured outpatient program for individuals who need support managing psychiatric conditions, navigating significant life changes, or rebuilding their lives after hospitalization, incarceration, homelessness, or substance use recovery. The goal is to help each client reintegrate into the community with the tools, confidence, and resources needed to thrive.

Who We Serve

We welcome adults who are:

• Living with mental health disorders such as depression, bipolar disorder, PTSD, or schizophrenia

• Transitioning back into the community from institutional care

• In need of support to rebuild daily life skills

• Seeking stability and connection after trauma or crisis

Our Services Include:

• Individualized care plans tailored to each client’s goals and abilities

• Life skills training (e.g., budgeting, meal planning, hygiene, job readiness)

• Medication management support and health education

• Therapeutic groups focusing on emotional regulation, coping skills, and peer support

• Community integration activities to foster independence and belonging

• Transportation assistance for eligible participants
